How do I write a professional email..?

I am a student.. and I am currently working for a charity program.. my job is to find sponsors for a trip to Hong Kong for charity work... I am suppose to send a very "very" professional email to a CEO of Toyota in China (She knows English). How am I suppose to sound professional when inquiring for telephone information, asking questions involving whether if they will decline or accept to be our sponsor and what our purpose is? Also, how should I introduce myself, should I tell him/her what I represent? and also, should I tell them who gave me their email?
