What the hell is this?

Can you explain this to me (I think they're trying to let me go):

This form was left for me to fill out (I'm leaving in three weeks anyway):

It asks for:

Employee Information-
Name, Social Security Number, Address, Date of Birth, Telephone

"Nature of Action" (Leaves room to check)
-New Employment, Regular, Part Time, Temp, Seasonal, Rehire, Transfer, Compensation Change, Name Change, Leave of Absence, Return from Absense, Resignation, Termination, Laid-off

"Payroll Data".

This form is a file to indicate Status Change, but my Status is not changing as of yet.....Here's my other question with details (Sorry I know this is confusing): http://answers.yahoo.com/question/index;...

Do they know you are leaving in 3 weeks? You could check the box that reads "Resignation" if you want, and enter the last date you intend to work. They might want to force you into an exit interview so watch your p's & q's. That exit interveiw could bite you in the butt later unless you are sweet as pie and tell them you just loved the job and thank them for the opportunity. I would put everything in the Emp Info area and leave that "Nature of Action" area blank. They should have told you what to do. Find the person that gave it to you and ask "What is this for?"

This Status change form must be filled out by you because you have gone from part-time to full time. Please do so promplty. Not to can mean loss in benifilts etc.
