Do not put responsibilities include but are not limited to...Just put Responsibilities and bullet point all the things you did, making them one sentence and to the point.

You need to be working with a wordprocessor to format everything in an attractive way. a say orginize all you work history into some sort of timeline Why can't you just say as the objective, Customer Service Representative? You should leave the kissing up stuff in your cover letter. Three things
1. One this is a place to ask questions you are confused about not a place to be lazy and let other people do your work
2. You know yourself better then anyone else so you should be doing your resume
3. What the duece is your question? It actually looks pretty good. Some spots you used the wrong wording but other then that is seems ok. toughest question to answer "why did you leave your last position. Resume will get an interview Your objective is too jargony and useless for one. The "references available upon request" is completely unnecessary and you need to put in accomplishments, not duties. In other words, omit "Responsibilities include but not limited to".
I would really need to tinker with this off-site in order to really help you. Your sentences go back and forth between present and past tense, there is too much jargon and fluff in it, grammar is lacking, too many highlighted duties [accomplishments], and you need to consider limiting all of these points to short bullet points using no more than 3-4 per section.
Try "Experience" over "Work History", "Qualifications" over "Skills". In education, you may want to try:

Your primary goal is to sum up YOU as simply and quickly as possible. The fluff and other nonsense will leave you hitting the trash bin.
