Issue of fresh passport after marriage??

Hi,
My wife had a passport before marriage. Now after marriage, we are thinking of getting following details changed -
1. Her name updated in her passport (due to Name change after marriage)
2. My name to be included in her passport in Spouse Name section
3. Address change

Do we need to go for Misc. Services application form (Application form 2) or need to apply on Application form 1? People at "May I help you" counter at Regional Passport Office are themself confused. Thanks in anticipation.

PS: Would Regional Passport Office issue a fresh passport of make changes in existing?

You will have to use Form 1. Check this page http://passport.gov.in/cpv/Forms.htm.

It says that
"Form No. 1 is to be used for Fresh/ Reissue/Replacement of Lost/Damaged passports/ Change of Name/ Appearance/ Exhaustion of pages . The same application form is used for passports for minors as well."

I suggest you and your wife go to your nearest U.S. Embassy. Whatever the Embassy officials tell you, get it in writing, and signed by an Embassy official.
