How much does it cost to rent a US Pos Office Box? What do you need to provide?

There were fees listed, but no indication of whether those were monthly or annual, as well as which fee group
I would be in.

You were looking at the correct information on the usps.com website. By reading the documentation at:

I found that the post office box fees are charged on a semi-annual basis. The document also explains that the fee groups are based on the zip code of your local post office, but says that you need to contact your local post office for that information.

Varies on the size and purpose of the box. Small boxes are very reasonable.
