we need the following:1. Copy of ID (ID Card, Driver鈥檚 License or Passport) of credit card holder and one additional copy of something such as a armed forces, university, government, company ID card, green card, lease or mortgage on your home, voter or car registration card, home or car insurance policy with your name on it. It does not have to be in the English language. 2. You will also need to download and fill out this form which authorizes us to pick up your mail for you from the post office. (We also need you to mail the original of this form 1583 back to us) (This form needs to be notarized unless you can stop at our office in person) Directions on filling out the Form 1583 can see seen here.3. Also, sign and fax or e-mail back this form. You can e-mail these 4 items to us or fax to + 1 310 854 5621.We will be able to activate and e-mail you your address later today and look forward to welcoming you as another of our --- based clients.

Fax them a Neat Letter, Explaining your needs.
& have someone help you write it good, it will cost you a buck or so for a Fax, but might be worth it,
