How to port foward my BTHome Hub for Call Of Duty 4?

I can't play call of duty 4 online because it keeps freezing up. Activision said that i need to forward ports on the router. They said that cod4 uses ports 3658, 3478, 3074, 3075 (UDP) and ports 80, 443, 5223 (TCP). But which one of that list to i forward the port to?

Open bTHome Hub in programs then open help then games, this is were you configure the hub for cod4.
