Voicemail Message for terminated employee?

I have an employee gave her 2 week notice and her last day is on Friday. I want to setup her voicemail message, so it states that she no longer works here and give them the extension of who to contact. I want to do the same thing with her email, so any clients that email her or call her will know who to contact.

I am struggling with how to word it.

I am planning on having her email and voicemails forwarded to another employee, but I want to have a message also so they know who their new contact person will be.

Hello,

As of (insert final date here), (Former employee name here) is no longer with (name of company). Please contact (name of contact person) for any concerns that you may have.
Thank you and have a great day.
