What should I put on a cover page of a business proposal to create a joint venture?

I am offering a proposal to the company that I work for but I'm not sure what information should be on the cover page, also I'm not sure about how it should look...
So the details: creating an exchange program through two large training centers
one in america, one in china.
Let me know, because I can't seem to make it look, or feel suitable

Well I wish I was that rich! but the place I'm offering it too is in China so all of that flowery stuff is sort of necessary. (Chinese Culture loves to slap people in the face with cheesy idealistic stuff.)

Put a copy of your last W-2 and the best looking bank statement from your IRA, that should do it, if you're earning attractively that is. Why beat around the bush with stupid mission statements that are both cheesy and idealistically phony? Just cut to the chase and let them know you have money, that's all they really want to know, in the end, correct?
