I'm completing an unclaimed property claim?

I am completing a unclaimed property claim in Wisconsin from a paycheck i did not collect a few years ago. My problem is they are asking for "evidence or my previous address OR documents showing I conducted business with the property holder" which I assume is the company I used to work for. How can I get either one of these (I no longer know how to contact my former landlord), i havent worked for that company in 3-4 years, and i havent lived at that address in about the same amount of time. Could I get this info from my bank somehow, like proof they used to send my statements to my old address? Thats all I could think of but any input or advice would be helpful. Thanks.

I believe you have answered your own question. :)

If you can obtain any proof from a reliable source like banking or electric bills, etc., with your name and address, you should be good to go. You may also obtain this info from credit reports as they should have every address you have ever lived.

Lastly, though you have not worked for the company mentioned in 3-4 years, they should still have your info on file which will also be helpful and proving your claim.
