Can someone translate an address to spanish format?

i have to make business cards for a fake business in my spanish class and the only day ive been absent all year she goes over how to write addresses in spanish speaking countries,
the address i need translated is

550 Madison Avenue
New York, New York

and how do you say fax in spanish?

550 Avenida Madison
Nueva York, Nueva York


(As far as I know, "fax" in Spanish is "fax"...)

I speak Spanish and I live in the U.S., I think I know how to translate an address...
I couldn't care less about the 10 points, but I know this is correct, you can be sure you can use this in your class, you won't have a problem...

You wouldn't translate an actual address, at least not for the US.

If you have an address in Puerto Rico, however, the address format will change. Take a look at this PDF file from USPS for an example.

You must follow precise standard formatting from USPS if you want to be sure your mail will get where it is going. I worked misdirected mail (and frequently undeliverable mail) for a long time and the SLIGHTEST error will get it returned.
