ATT Help? PUK, PIN, Smartchip Rejected [Read Inside]?

Okay--
My friend's phone locked her out after she tried to guess her PIN. After so many tries, it started to ask for a PUK code (which is, what, by the way?). Neither the PIN nor the PUK code would go away when we tried to exit it. When we failed to correctly guess that, it said (-and still does when we turn it on), "smart chip rejected", and she can no longer use her phone. We tried to contact ATT, but the customer service number is very hard to locate. If you cannot answer our main question, can you give us the number to call (1-800 number, if possible).

kate the PUK is a pin unblock code and is avail from ATT customer care or from the www.wireless.att.com website once you sign in.
Once entered too many times incorrectly the sim card is burned out you friend will need to purchase a new sim card they run about $25 bucks and are available anywhere ATT service is sold.
For future reference customer care number is 1-800-331-0500 or 611 from an ATT phone.
